Insurance script

Prior to your initial appointment, please verify coverage of nutrition counseling sessions. This benefits both the clinician and you as the client. If we meet for a nutrition counseling session and then find out that insurance denies coverage, then you are financially responsible for that denied claim. Below you will find an insurance script to help guide you through the process of calling your insurance provider, as well as information that the representative may seek.

Call the member services number on the back of your insurance card and ask: 

  1. Does my plan cover outpatient nutrition counseling? 

    • If yes, how many visits are covered?

    • Does my plan only cover visits that are “medically necessary”?  

  2. Does my plan cover telehealth or telemedicine visits? Does coverage only apply to the COVID-19 pandemic or will it be covered beyond as well?

  3. Do I have a deductible to meet first?

  4. Do I need a physician referral?

    • If you need a referral, this must be done prior to your initial appointment

  5. What is my co-pay amount for outpatient nutrition counseling?

  6. Record the representative’s name and a reference # when checking your benefits. This information will be necessary if you ever need to dispute a rejected claim
